Answers: 1) if after u put it within u feel like u hold to waddle around, then its not high plenty. It should be comfortable, u should't even know that it is there. sometime u can feel it dance over a ridge when inserting it so then u know its up
2) no it can't come loose, and even if it did u could just stick ur fingers surrounded by and get it, but it can't so don't worry
1) u will know if it isnt, it will be mortified. It shouldn't feel like anything when its contained by properly, and you really dont have enough room up nearby for it to go any farther than it should.
2) no, the string won't come loose. I have never hear of that happening. But if you are paranoid about it a moment ago get (a fresh) one and yank on the string, you will see that it is attached all right and the string actually is part of the tampon, it go all the way through the spongy cottony part.
In every box of tampons there is even a piece of paper/instructional sheet that have a little drawing that can explain insertion the best. So if you do decide to use tampons after read the little package insert.

you really only push it up inside you as far as you can. you'll know if it is too far, but like i've never had it shift up insid me too far.
as many times as i've put a tampon inside me, i've never had a string break on me or anything similar to that happen to where i have an issue with getting it out.
