The closer they are the more medical problems. Since so much of the DNA will be similar in the child near will be more defects and errors contained by the code.

Step siblings would probably be safest, since the blood/genetic relationship is plausible very diluted (unless you're, approaching Rudy Guliani, or something).

Legally, incest is defined as a sexual relationship between two folks related by blood or marriage - technically making a sexual relationship next to your step-sibling incestuous. This definition may vary from state to state, but mostly, that definition pertains to all US states.

The risk of birth deformity is very large among people who practice incest because their DNA carry the same genes, creating a substantially greater probability of a particular birth malfunction being passed on. It's not guaranteed - but much more expected than if 2 strangers mate. The further removed genetically from your partner, the less predictable that particular birth fault would be passed on. Having a child with your second cousin, afterwards, would be risky, not to mention ethically questionable. Either way, it's going to gross family reunions exceptionally uncomfortable.

Reference the X-Files episode where on earth 3 brothers had sex beside their mother and produced a child (who died) who displayed nearly every genetic birth defect set to modern science. This family have a history of incest between siblings and parents, apparently, and that was the result (ick!). Fictionally extreme, but scientifically possible considering the sexual activities of that out of the ordinary group.

Your step-sibling - not a half sibling - is probably past the worst. I do know a couple who met, married, and later his widowed father married her divorced mother. This made them step-siblings after their matrimonial. Naturally, their children were wonderful and defect free, but technically, they be breaking the law by committing incest.
