I am no doctor, but I imagine you may be trying too hard or thinking more or less it too much. I used to have discomfort when I orgasmed because I get so damn tense when I be trying. And for me, half the time it didn't up self that great. Try getting massaged or getting reverberatingly relaxed before appendage if that's a possibility. I don't think it's medical.
