Because it is a medication that can caused serious problems if used incorrectly. Just similar to all other medication that are dangerous if used incorrectly, birth control requires a prescription.
I don't know if this is the correct answer or not, but some society have conflicting form issues such as migraines. On pills, patch, or ring, if they start to get a fruitless migraine, the doctor's can stop the medication. Not so with the shot.
Besides, a condition care provider's gotta inject you. They know how much to donate you and won't mess up giving you the shot (no air embolisms).
