With transvaginal ultrasound, the earliest sign of pregnancy would be a gestational sac inwardly the uterus. Under optimal conditions, we could see this in the 4th week of pregnancy, but sometimes, it is not seen until the 5th week. The yolk sac can be see in the 5th week and the fetal pole in the 6th week.

"The gestational sac is the earliest sonographic finding surrounded by pregnancy..The gestational sac first appears at about 4 weeks gestational age..While a gestational sac is sometimes seen as precipitate as during the 4th week of gestation, it may not be seen until the end of the 5th week, when the serum HCG level have risen to 1000-1500 mIU.The yolk sac first appears during the fifth week of pregnancy.A mass of fetal cells, separate from the yolk sac, first become apparent on transvaginal ultrasound just after the 6th week of gestation."
